Please follow our simple Instrument Service and Repair Instructions to ensure all repairs are processed correctly and in a timely manner.
- RMA numbers are required for all repairs. Any instrument
sent to Miltex, Inc. without an RMA number will be returned to the sender. It will not be evaluated or repaired.
- Used instruments must be cleaned and sterilized prior to
sending them to Miltex, Inc. for repair.
- Contact our Customer Service Department at 1-866-854-8300 or email us at customerservice@miltex.com. Please have the following information available.
- Account name and shipping address
- Contact name and phone number
- Part number and quantity
- Description of required repair
- Payment method
- Distributors will need their customer account number and a valid purchase order number authorizing the repair service.
- End user facilities, such as Doctors, Dentists, Medical-Surgical Centers, should be prepared to make payment by credit card.
- Wrap instruments, especially sharps, to protect them during transit. Use a common carrier such as Federal Express or UPS and insure the package. Keep a record of the tracking number.
- RMA's not received within 30 days will be cancelled.
